Offside Rear inner part of disc brake disc in such a condition that it is seriously weakened (3.5.1i)

Offside Rear Rear brake application uneven (3.7.A.5c)

What do I need to do to fix this? Replaced the disc today but unsure what exactly rear brake application uneven refers to?
Cheers.

Slider pins were good, let's just hope it was something to do with the discs. Did pads at the same time too.

check the release spring arms for the handbrake they occasionally partially seize, resulting in binding and rapid decay of rear brake parts.

Just for an update I passed the MOT today all good. All I did was change the rear discs and pads, made sure all the sliders were working and wired brushed where the pads sit. Thanks for your help. Also make sure you go to a council MOT center they are awesome.
